I've never had any mishaps worth nothing, the closest I can come is when I made soup one time and put it in a thermos flask and the glass bulb inside it somehow broke

I watched someone else once use (on purpose) dirt in soemone's food (needless to say they were not happy with the person.)

Made a near perfect loaf of bread for Home Ec.... save for the fact that I forgot to put salt in it.

I friend tried to make me a birthday cake on my 18th with laxettes in the icing. It was pretty obvious.
I think i donated it to meals on wheels.

Actually i do a roast each week and a couple of weeks back i decided i would add yorkshire puddings to the plate.
I called mum to ask her for the recipe and she said that they were really a basic pancake mix. She would assume of course that i would follow a recipe and just not add sugar.
If i had followed a recipe i dare say i would not have added sugar but been gifted with both a sense of adventure and complete laziness i hit the supermarket and came home with a bottle of shake and bake pancake mix.
Sweet puddings and gravy don't really go.

Back in my college days, two buddies and I tried to 'cook' some homebrew using a rice/potato mash. The resulting product did burn, but it tasted like turpentine and smelled worse. Had one friend from high school come by for a party and he actually liked it. He was probably stoned on something, maybe that accounted for it!
