My guess is Worf's condition is left ambiguous on purpose, since it give maximum flexibility to any possible continuation of the story. The mark of a 'cliffhanger' ending, to be sure.

Worf will probably appear later on in a post-TNG novel, or maybe as a cameo in a future TV series. Just my guess.
