In the opening theme of Enterprise we see another ship prior to the final shot of the NX01. Its only the rear end showing the nacelles and impulse engines. Its not the NX01 because the configuration is different. It's obviously the model prior to the NX01

Do you know what ship/model it is?

It was just a rendered back of a ship. The rest of it was undetailed/unsurfaced. Later, the lunar ship's CGI model was rerendered and complexified to become the Warp Delta types seen in the finale of Season 2, "Twilight" in Season 3, and the Mirror episodes of Season 4

Wasn't one of these ships the Intrepid?

The Intrepid was in Twilight.
But the ship in the opening shot is the Warp Delta type. Beautiful shot, Glacial.

My theory is that the lunar ship/warp delta ship did indeed look like that from the back at one time prior to the NX-01's construction, but was later refitted to the variant that we would later see in "Twilight". Seeing as, of course, both ships have very different aft sections.

Actually that was an early concept drawing done by Matt Jefferies for Star Trek: Phase II to redesign the Enterprise. You can find the Drawing in a book called The Art of Star Trek By Judith and Stephen Garfield

Have the book. But, the only illustration that looks remotely like it is the sketch drawn by Ralph Macquarie?

Looks a little like a different take on an TOS Romulan Warbird.
