I never saw Janeway as being a motherly figure. Even if she cared for her crew in a motherly fashion, she cared about the chain of command and would never allow herself to be too "motherly" to anyone other than a few of her senior staff. She was obviously very open with her own thoughts and feeling to Chakotay. She was also a mother figure to Seven and also Harry, but beyond that....?

I think you are right about the fact the crew was probably not very comfortable with the doc early on. I always thought of Kes as the person the crew would talk to since she was working in sickbay. After her departure I believe the crew would have been comfortable with the Doc by then.

Ya got that right about doctors onboard ships. Hell, I get so much grief from my own patients that after 10 years in practice as a heart specialist in New York City, I should be allowed to sit for phychiatric Board Certification.

Im pretty sure there will be no such person aboard 1701. Bones was always fullfilled that role aboard the Enterprise. Id hate it if there was one added to TOS crew. no point and no need.

I reckon there probably would be a counsellor or psychotherapist on the ship, just not as a main or pivotal character like Troi was.

There are just over 400 people on the ship...pretty much everyone has to pull two or three jobs. McCoy would, naturally, also be the ship's counselor. I'd bet that Chapel and M'Benga had lesser training but were also qualified to act in that regard.

The ship's probably full of people who we can 'assume' are there, even though we never see them in any episode or film: the cleaners, most of the technicians and lab scientists, the quartermaster... sure they pop up from time to time, but usually they're invisible. I figure the 1701's mental health team would be like that. It'd make sense for them to have one - keeping everyone healthy and happy during a long voyage in a confined, sensitive environment would be an important consideration.

Troi was on the bridge crew of the Enterprise-D mainly because the 1980s were the golden age of professional therapy, when it was totally in vogue for the rich and powerful to be in therapy. Now, counseling is just another part of general health and wellbeing, often enough, so having the ship's counselor as a central character seems less likely. Troi's empathic ability made her a good person to have on hand in a crisis, though, so her shifts on the bridge are logical and understandable.

....400 people is a parking lot(meaning, tiny)...they all had to do extra duty so ascribing "counselor" duty to the Doctor not only follows canon, it makes sense.

We never, ever, ever saw any suggestion that there was someone else they might "talk too".
